Am I a Candidate for a Tattoo?

(This applies to both body tattoos and cosmetic tattoos.) Pre-assessment is crucial for the best results.
Tattooing is not suitable for everyone. Your safety, healing, and results depend on your overall health and skin condition. Please review this checklist before booking. If any apply, you may not be a candidate without a doctors note; Please inquire if you had any questions.

General requirements for all tattoos:

 ☐ My skin at the tattoo site is healthy (no rash, acne, cuts, or irritation).
 ☐ I can follow aftercare (no swimming, tanning, saunas, or heavy sweating for 1–2 weeks).
 ☐ I will bring a valid government-issued photo ID to my appointment.
 ☐ I understand a doctor’s note may be required in some cases.

I cannot be tattooed if I:

 ☐ Am under 18.
 ☐ Am pregnant or breastfeeding.
 ☐ Am taking blood thinners (Coumadin, Eliquis, Xarelto, Pradaxa, heavy aspirin), fish oil, krill oil, vitamin E, ginkgo biloba, or other supplements/medications that thin blood.
 ☐ Am taking immunosuppressants (including steroids) or have an immune disorder without clearance.
 ☐ Am under the influence of alcohol or recreational drugs.
 ☐ Have a history of alcohol or substance abuse.
 ☐ Have uncontrolled diabetes (especially insulin-dependent without clearance).
 ☐ Have untreated or undiagnosed heart conditions, or a pacemaker without clearance.
 ☐ Require antibiotics before dental work due to infection risk.
 ☐ Have an active infection (cold, flu, shingles, cold sores, MRSA, skin infection at/near the site).
 ☐ Have a history of keloid or hypertrophic scarring.
 ☐ Have autoimmune skin conditions (eczema, psoriasis, lupus, rosacea) that are active in the area.
 ☐ Am unwilling or unable to follow aftercare instructions.
 ☐ Am actively tanning or my skin is sunburned/sensitized.

I may need to wait if I have:

☐ Taken antibiotics in the past 2–4 weeks.
 ☐ Had Botox or fillers in the last 3–4 weeks.
 ☐ Had chemical peels, microneedling, or laser in the last 3–12 weeks.
 ☐ Had cold sores or shingles in the last 6–12 weeks.
 ☐ Am recovering from surgery (must wait at least 4 weeks).
 ☐ Have had chemotherapy or radiation (must wait 6–12 months with doctor’s clearance).
 ☐ Have recently stopped Accutane (must wait 12 months after last dose).

Cosmetic tattoo–specific notes:

☐ For lip blush: I understand I should ask my doctor about antiviral medication if I have a history of cold sores.
 ☐ For brows/eyeliner: I will discontinue lash or brow serums 3–6 months before my appointment. Avoid tinting 3-4 weeks prior to analyze natural hair color.
 ☐ I understand trichotillomania (hair pulling) may prevent me from getting brow tattooing.

Previous cosmetic tattooing (brows/lips):

☐ I will email clear, makeup-free photos before booking.
 ☐ I understand old pigment must be ~80% faded for new work.
 ☐ I may be advised to wait longer or pursue laser removal before booking.
 ☐ I understand booking without approval may result in forfeiture of my deposit.
